Amongst other methods, this study used two questionnaires: a detailed one for deaf children, and a briefer one for service providers. These are reproduced in full in the appendices, and their design and use is described in the methodology section of the report.

A report on a web questionnaire used to consult over six hundred children on the subject of school meals. In particular, look at the appendix, which contains the full questionnaire.

International and Internet Cases of Child Sexual Abuse: Research Materials

This biog of Bernard Gallagher on the University of Hudderfield site contains links to resources for one of his current projects: two questionnaires and guidance notes for their completion. Look also at the research summary, which details the background and methodology. Note that this is a study about children, but which does not engage with them directly. What do you think of the methodology? Could it be changed to consult the children involved, or would this be unethical in the context?

There are plenty of sites which explain statistical concepts, but most of them will probably confuse you further. However, this page is both comprehensive and comprehensible, should you require assistance with the dreaded stats. Its location on the server of a US children's health care organisation is purely coincidental.
